What is Xtandi?

Xtandi is a brand-name prescription drug that treats castration-resistant prostate cancer, that is, prostate cancer that has not progressed using therapy to reduce male hormones (androgen depletion therapy). In prostate cancer, the uncontrolled growth of prostate cancer cells is caused by male hormones attaching to proteins—called androgen receptors—on the surface of the cancer cells that then “signal” them to divide into new cancer cells.

In castration-resistant prostate cancer, the cancer cells have so many overactive androgen receptors that even severely cutting back on the amount of male hormone in the body doesn't stop the growth of the cancer cells. The active ingredient in Xtandi, enzalutamide, attaches to those androgen receptors instead. Because male hormones are not able to attach to these receptors, the cancer tissues stop growing.

Xtandi is taken as a capsule once daily.

What are the side effects of Xtandi?

Common side effects of Xtandi include, but are not limited to, joint pain, muscle pain, back pain, hot flashes, headache, dizziness, frequent urination, diarrhea, swelling, insomnia, feeling tired, fatigue, and cold symptoms. Mental and mood changes include anxiety.

Serious side effects of Xtandi include high blood sugar, heart problems, seizures, and lung infections. Among the most severe side effects of Xtandi are bleeding problems (nosebleeds, blood in the urine), which include a rare condition called posterior reversible leukoencephalopathy syndrome (PRES), a potentially life-threatening condition in which capillaries in the brain “leak.” This syndrome can happen even after Xtandi has been discontinued. Symptoms of PRES include headaches, vision problems, seizures, and changes in mental functioning. Report these immediately to your physician.

Allergic reactions are rare. Watch for important signs of an allergic reaction, including swelling, itching, or rash, and talk to your doctor if you experience what you believe to be an allergic reaction.
