What is Xgeva?

Xgeva is a brand-name prescription drug used to prevent bone loss in patients with multiple myeloma, giant cell tumor of bone, or other cancers that may cause bone loss. Multiple myeloma and giant bone cell tumors almost always involve bone loss. Xgeva can also be used to treat high blood levels of calcium (hypercalcemia) in people with cancer. 

Multiple myeloma is a cancer of the plasma cells, a type of white blood cell. Cancerous cells can gather in the bone marrow and become more abundant than healthy cells. Multiple myeloma cancer can increase the activity of osteoclasts, or bone cells responsible for breaking down bone , which can increase the release of calcium from the bone into the bloodstream. This can lead to high calcium levels and soft spots in the bones. The bones can gradually weaken and become more vulnerable to fractures.

Giant cell tumor of bone is a rare and benign tumor that forms at the ends of the long bones of the leg. Although the tumor is benign and almost never spreads to the rest of the body, it is locally aggressive and actively removes calcium from the bone around it.

High blood calcium due to cancer (hypercalcemia of malignancy) is a feature of many cancers and can have serious consequences. 

Xgeva, or denosumab, is a RANKL inhibitor. TWhen it is injected into the body, it attaches to a protein, called RANK ligand, that is found on certain bone cells that reabsorb calcium from the bones (osteoclasts). Denosumab, also considered a monoclonal antibody, targets RANKL and blocks its actions to prevent the breakdown of bone tissue and the release of excess calcium in the blood. 

Xgeva is usually administered as a subcutaneous (beneath the skin) injection once every four weeks.

What are the side effects of Xgeva?

Many people may experience injection site reactions, such as pain, redness, or irritation around the area of injection. In general, common side effects of Xgeva include, but are not limited to, headache, joint pain, back pain, muscle pain, diarrhea, nausea, weakness, and feeling tired. Serious side effects may include death of bone cells in the jaw (osteonecrosis of the jaw), unusual fractures of the femur, and low blood calcium levels (hypocalcemia), which could be life-threatening. Tell your physician if you notice worsening or persistent side effects. 

Xgeva is toxic to fetuses, so consult a healthcare provider before starting Xgeva if you are pregnant.

Allergic reactions are rare but can be severe. Signs and symptoms of an allergic reaction may include rash, swelling of the face or throat, and trouble breathing.
