Symptoms of a urinary tract infection include an urgency to urinate, urinating more frequently, a burning feeling when you urinate, or blood in the urine. Genital fungal infections (such as vaginal yeast infections) can also occur. Contact your doctor if you experience any of these effects and they persist or worsen.

Alcohol can interact with this medication. Talk to your healthcare provider before taking Synjardy XR if you drink alcohol. 

Individuals who take a diuretic, such as hydrochlorothiazide or Lasix (furosemide) are at higher risk for dehydration when taking Synjardy XR. In these patients, kidney function and volume status will be closely monitored.

This medication generally does not cause hypoglycemia (low blood sugar), but hypoglycemia may occur if you are using this drug with certain other diabetic medications. This includes insulin or certain diabetes medicines such as Amaryl (glimepiride), glipizide, or glyburide. 

On the other hand, certain drugs tend to increase blood sugar and may require dosage adjustments and/or close monitoring. Examples include diuretics, steroids such as prednisone, thyroid medications, oral hormonal birth control, Dilantin (phenytoin), and calcium channel blockers.

What happens if I miss a dose of Synjardy XR?

Synjardy XR is not appropriate for everyone. Some people cannot take Synjardy XR, including:

• People who are allergic to metformin (also known as metformin hydrochloride), empagliflozin, or any drugs in the SGLT2 inhibitor drug class

• Women who are pregnant (second and third trimester)

• People with type 1 diabetes

• People in metabolic acidosis or diabetic ketoacidosis (DKA)

• People with hypoxemia (low level of oxygen in the blood)

• People with volume depletion (low fluids) or dehydration

• People with liver problems or severe kidney problems

• People who are septic

• People who are having surgery

Synjardy XR should not be used in the second or third trimester of pregnancy because it can cause harm to the unborn baby. Synjardy XR is not recommended for use in breastfeeding women. Women who are pregnant or planning to become pregnant should consult their healthcare provider.

Talk to your healthcare provider about how and when to check your blood sugar, and what to do in the event of low blood sugar. Ask if you should carry emergency glucagon, which can be used in a low blood sugar emergency. Your pharmacist can show you how to use it, and you can teach your family, friends, and/or caregivers how to administer it to you if there is a low blood sugar emergency where you cannot talk or eat. You should also wear a medical identification that lets emergency responders know that you have Type 2 diabetes mellitus.

If at any time you are sick with vomiting and/or diarrhea, notify your healthcare provider, because this might affect your blood sugar. Also, let your provider know any time you are scheduled for surgery.
