What is Rinvoq? 

Rinvoq is a prescription medication classified as a Janus kinase (JAK) inhibitor. Rinvoq is made by the pharmaceutical company AbbVie, Inc. It contains the active ingredient upadacitinib and works by suppressing the immune system. This medication is generally used when other medicine or previous treatment has not worked or cannot be tolerated. It is FDA-approved to treat:

  • Moderate to severe rheumatoid arthritis in adults

  • Active psoriatic arthritis in adults

  • Moderate to severe atopic dermatitis in adults and adolescents 12 years and older

  • Ulcerative colitis in adults

  • Crohn’s disease in adults

  • Ankylosing spondylitis in adults

  • Non-radiographic axial spondyloarthritis in adults

Before taking Rinvoq, tell your doctor about all your medical conditions, especially if you have (or have a history of) a weakened immune system, diabetes, lung problems, ulcers, diverticulitis, cancer, heart problems, heart attack, stroke, blood clots, liver problems, or low blood cell counts. Also, tell your healthcare provider if you smoke or if you have an infection such as HIV, hepatitis, tuberculosis, or shingles.

Rinvoq is taken by mouth. Swallow the tablet whole—do not crush, chew, or break the tablet. You can take Rinvoq with or without food, but take it at the same time every day with a full glass of water.

What are the side effects of Rinvoq?

Rinvoq has a black box warning, which is the most serious warning required by the FDA. The warning states:

  • There is a higher risk of serious infections when taking Rinvoq, which could lead to hospitalization or death. These infections may include tuberculosis or other types of bacterial, fungal, or viral infections. Avoid people who are sick, and let your doctor know right away if you have signs of an infection such as cough, fever, or lumps in the neck, armpits, or groin. Your doctor will monitor you for tuberculosis before and during treatment with Rinvoq. Your doctor will also order blood tests on a regular basis. 

  • There may be an increased risk of death, including sudden cardiovascular death, in patients who are 50 years of age or older, have rheumatoid arthritis, and at least one heart-related risk factor such as high blood pressure. There may also be a higher rate of heart attack, blood clots, or stroke.

  • There is an increased risk of certain cancers in people who take Rinvoq, such as lymphoma or skin cancer. 

Common side effects may include:

  • Nasal congestion (stuffy nose)

  • Runny nose

  • Nausea

  • Headache

  • Acne

  • Weight gain

  • Muscle pain

  • Bumps or pimples around the hair follicles

In addition to the black box warning above, other serious side effects may include:

  • Allergic reaction (get emergency medical help if you have hives, difficulty breathing or swallowing, or swelling of the face, lips, tongue, or throat)

  • Fatigue, pale skin, difficulty breathing

  • Gastrointestinal perforation (a hole or tear in the stomach or intestines): symptoms may include severe stomach pain, fever, diarrhea

  • Sudden vision changes

  • Increased cholesterol

  • Reactivation or worsening of hepatitis or shingles

Does Rinvoq cause weight gain?

Weight gain is a possible side effect of Rinvoq. In clinical studies, about 2% of people who took Rinvoq reported weight gain.  

Are there any known interactions with Rinvoq? 

Before taking Rinvoq, tell your healthcare provider about all the medications you take, including prescription and OTC drugs, vitamins, and supplements. Some examples of interactions include:

  • Antifungals such as ketoconazole or Diflucan (fluconazole)

  • Biaxin (clarithromycin)

  • Dilantin (phenytoin)

  • Grapefruit and grapefruit juice (avoid all grapefruit products while taking Rinvoq)

  • Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or Celebrex (celecoxib)

  • Rifampin 

  • Steroid medication such as prednisone

While taking Rinvoq, do not get any live vaccines, such as:

  • Measles, mumps, and rubella (MMR)

  • Rotavirus

  • Typhoid

  • Yellow fever

  • Varicella (chickenpox)

  • Shingles

A serious infection could occur if you receive a live vaccine while taking Rinvoq.

Are there any known contraindications with Rinvoq? 

You should not take Rinvoq if you are allergic to upadacitinib or any ingredient in Rinvoq, or if you have severe kidney problems. Also, you should not take Rinvoq if:

  • Your absolute lymphocyte count is less than 500 cells/mm^3

  • Your absolute neutrophil count is less than 1000 cells/mm^3

  • Your hemoglobin level is less than 8 g/dL

Also, Rinvoq may harm an unborn baby and should not be used if you are pregnant. While taking Rinvoq (and for some time after the last dose), you will need to use effective birth control so that you do not become pregnant. You should not breastfeed while taking Rinvoq and for at least 6 days after your last dose. 

Which is better Rinvoq or Dupixent? 

One clinical trial compared Dupixent (dupilumab) to Rinvoq for eczema. The researchers found that Rinvoq was more effective, but there were more safety issues (side effects) to consider. Because there are various possible uses for each medication and many individual factors to consider, the best way to determine which medication is best for you is to consult your healthcare provider, who can take into account all of these factors and determine the appropriate treatment for you.
