Xeljanz is a prescription drug in the Janus kinase (JAK) inhibitor drug class. It treats adults with moderate to severe rheumatoid arthritis, active psoriatic arthritis, moderate to severe ulcerative colitis, or ankylosing spondylitis. It can also be used to treat children with juvenile idiopathic arthritis. Xeljanz is generally used when other medications have not worked or cannot be tolerated.
There is also an extended-release version of Xeljanz called Xeljanz XR. Taken orally, Xeljanz blocks the activity of certain enzymes in the body that can affect the function of your immune system. Follow your healthcare provider’s dosage instructions.
It is important that you inform your healthcare provider if you are using any of the following medications: fluconazole, ketoconazole, methotrexate, rifampin, or any other medicines when using Xeljanz that affect your immune system (including azathioprine, cyclosporine, tacrolimus). You should also avoid grapefruit juice and other grapefruit products while taking Xeljanz.

Rheumatoid arthritis: In one study, adults with moderate to severe rheumatoid arthritis (RA) that could not tolerate other prescription medications (or other medications were not effective) received Xeljanz or placebo (inactive medicine). After 3 months, 59% of people who took Xeljanz reported an improvement in RA symptoms. Many patients reported they could more easily complete daily activities such as getting dressed and had less joint pain and swelling.
Psoriatic arthritis: A study in adults with psoriatic arthritis concluded that Xeljanz reduced joint pain and swelling by two weeks, although for some patients, results took up to 6 months. Half of the people who took Xeljanz reported less joint pain and swelling at 3 months, and 68% reported less joint pain and swelling at the one-year mark.
Ulcerative colitis: In a study of adults with moderate to severe ulcerative colitis, some patients reported less rectal bleeding and stool frequency at two weeks. Eighteen percent of people who took Xeljanz achieved remission at 8 weeks. Forty one percent of people who took Xeljanz 10 mg twice daily and 34% of those who took 5 mg twice daily were in remission at one year.
Because results vary, it is important to talk to your doctor about the benefits and risks before taking this medication.
Patients being treated with Xeljanz may be susceptible to serious infections causing hospitalization or death. Tuberculosis and bacterial, invasive fungal, viral and other opportunistic infections have occurred in patients using Xeljanz. Should a serious infection develop, you should talk to your healthcare provider about discontinuing Xeljanz until the infection is controlled and alleviated.
You should seek emergency medical attention immediately if you experience any allergic reaction such as hives, difficulty breathing, or swelling of the face, lips, throat, or tongue. You should also get emergency medical help if you experience sudden shortness of breath, chest pain that spreads to the neck or arms, nausea, severe dizziness, a cold sweat, weakness on one side of the body, slurred speech, or pain, redness, or swelling in an arm or leg.
Notify your healthcare provider immediately if you have a fever, night sweats, tiredness, weight loss, abdominal pain, diarrhea, wheezing, severe cough, or lumps in the neck, armpits, or groin.
