Xarelto is an anticoagulant medication or anti-clotting medication. This blood thinner can prevent and treat blood clots in adults, lowering your risk of stroke, deep vein thrombosis (DVT), pulmonary embolism, and similar conditions.
Your doctor may prescribe Xarelto if you have an irregular heartbeat (also called atrial fibrillation). You may also be prescribed Xarelto as you recover from hip or knee replacement surgery. Following these surgeries, a blood clot may develop in your leg, causing deep vein thrombosis. The clot may travel to your lungs, increasing your risk for a pulmonary embolism.
Xarelto is available in 2.5 mg, 10 mg, 15 mg, and 20 mg tablets. It is also available as an oral suspension. Xarelto may be taken by mouth once or twice a day, depending on the indication. Always follow the medication guide and your doctor's specific medical advice, as your medical condition and response to treatment will be important factors in determining your dosage. If you have been prescribed a stronger dose of Xarelto, your doctor will likely advise you to take this prescription medication with food. Ask your health care provider or pharmacist what to do in the event of a missed dose, because it will depend on the dose and frequency you take Xarelto. Xarelto should be stored at room temperature, away from heat, light, and moisture. Keep out of reach and out of sight of children and pets.
Some possible side effects of Xarelto include easy bruising and minor bleeding, such as nosebleeds. More serious side effects of Xarelto that require medical attention are swelling; prolonged, persistent, or unusual bleeding from cuts, nosebleeds, menstruation, or gums; dizziness or fainting; tiredness or weakness. If you have symptoms of a severe allergic reaction (hives, difficulty breathing, swelling of the face, lips, tongue, or throat) or a severe skin reaction (red or purple rash, blistering or peeling skin, burning eyes, sore throat, fever), get emergency medical help. You should always contact your healthcare provider with any issues or questions about Xarelto. Before taking Xarelto, read all of the patient leaflets for important information about this drug’s side effects, warnings, and more. Note: This is not a complete list of possible side effects.
According to the American Red Cross, you should not donate blood if you take a blood thinner like Xarelto. This is because your blood will not clot normally. If your healthcare provider tells you to stop taking Xarelto, you should wait at least 2 days before donating blood.
Xarelto belongs to the drug class called factor Xa inhibitors, which is a type of blood thinner. Although rare, if you experience uncontrollable bleeding while taking Xarelto or another factor Xa inhibitor, such as Eliquis, your doctor may use Andexxa to reverse the effects of Xarelto. Andexxa is the first factor Xa inhibitor antidote. It was approved by the FDA and released in the United States in May 2018.
The following is a list of some possible interactions that could affect how Xarelto works or worsen its side effects:
Alcohol may increase the risk of bleeding from Xarelto and should be avoided.
Aspirin is another blood thinner that could increase your risk of bleeding while taking Xarelto (although some individuals on lower doses of Xarelto take a low dose of aspirin as well; your doctor will tell you if you should take aspirin or avoid it)
Certain antidepressants, as well as antifungals (such as ketoconazole and itraconazole) may slow down the removal of rivaroxaban from your body, causing Xarelto to build up and cause more side effects like bleeding.
Avoid grapefruit products while taking Xarelto. Grapefruit and grapefruit juice can affect how Xarelto is metabolized by the body and can increase Xarelto levels in the body, which can cause serious side effects such as bleeding.
While many drugs cause a buildup of Xarelto, on the other hand, many seizure medications speed up the removal of rivaroxaban from your body, making Xarelto less effective.
Before taking Xarelto, tell your physician about all of the medicines you take, including prescription and OTC medications, vitamins, and supplements. Your physician can provide additional information about drug interactions.
Xarelto has gained popularity over Coumadin (warfarin) as a newer and slightly safer alternative. They are both used to prevent and treat blood clots that could result in stroke and heart attacks. However, Xarelto has a lower risk of causing a stroke or bleeding, particularly in the brain. Warfarin also requires regular blood tests to monitor the patient’s response, while Xarelto does not.
Examples of other anticoagulants include Eliquis, Savaysa, and Pradaxa. Your doctor will determine which prescription medication is better for your medical condition based on full prescribing information.
Generic Coumadin (warfarin) is an inexpensive alternative to Xarelto and is covered by most commercial insurance companies. However, it comes with a higher risk of bleeding and the need for frequent monitoring. Another factor Xa inhibitor may be a good option if your insurance does not cover Xarelto or charges a high copay. You can ask your insurance provider about their health plan requirements and if they cover another factor Xa inhibitor such as Eliquis (apixaban) or Savaysa (edoxaban). These drugs are all brand names and are not available in generic at this time.
Can you take Tylenol with Xarelto?
Tylenol (acetaminophen) is generally safe to take with Xarelto. Most nonsteroidal anti-inflammatory drugs (NSAIDs), like ibuprofen or naproxen, should not be taken with Xarelto because the combination can increase the risk of bleeding, which could be life-threatening.
However, in some cases, Xarelto is taken along with a low dose of aspirin. Be especially careful when taking cough/cold medications because they may contain NSAIDs. Always consult your pharmacist or healthcare professional if you are unsure about taking medications together.
