What is Qsymia?

Qsymia is a brand-name prescription drug used to promote weight loss when used with an appropriate diet and exercise plan. It is approved for people with a body mass index of greater than 30 or for people with a body mass index greater than 27 and certain health risks, such as high blood pressure, diabetes, or high amounts of fat in the blood.

The active ingredients in Qsymia are phentermine and topiramate. Phentermine is an appetite suppressant or anorectic that supports weight loss by reducing appetite. Topiramate is an anticonvulsant that has long been used to prevent seizures in patients with epilepsy as well as prevent migraines. Topiramate changes the chemistry of the brain to reduce nerve activity. How it works to produce weight loss is not well understood.

One of the most common side effects of topiramate is weight loss, so it has a history of being prescribed off-label to aid in weight loss. Qsymia is the first FDA-approved use of topiramate as a weight-loss drug. However, topiramate may cause serious side effects and cause birth defects. For this reason, Qsymia is only available through a REMS (Risk Evaluation and Management Strategy) program and sold only through certified pharmacies.

Qsymia comes as an extended-release formulation in four dosage strengths. It is taken as a capsule once daily without food.

What are the side effects of Qsymia?

The most common side effects of Qsymia include but are not limited to dizziness, change or loss in taste, dry mouth, trouble sleeping, constipation, numbness or tingling in the limbs, depression, joint pain, fever, and flu symptoms. The most serious but rare side effects of Qsymia include fast heart rate, eye problems such as glaucoma, and suicidal thoughts or behaviors. 

Other side effects of Qsymia include mood changes like depression, problems with concentration or memory, increased levels of acid in the blood (metabolic acidosis), kidney problems, kidney stones, and skin reactions. Qsymia may also cause low blood sugar levels (hypoglycemia) in people with diabetes or low blood pressure (hypotension) in people taking blood pressure medications. 

Qsymia may cause a cleft palate in a fetus when taken during the first three months of pregnancy. For this reason, Qsymia is prescribed through a REMS (Risk Evaluation and Management Strategy) program and is available only at certified pharmacies. Although not required, women of childbearing age may be advised to have monthly pregnancy tests and to use birth control.

Allergic reactions are rare. Discontinue Qsymia if there are signs of an allergic reaction such as hives, rash, or trouble breathing.

Is Qsymia better than phentermine?

Qsymia can be more effective for chronic weight management than phentermine alone, as it combines phentermine with topiramate. These two active ingredients work together to effectively promote weight loss. However, Qsymia may cost more and have more side effects than phentermine alone.
