Xdemvy (lotilaner ophthalmic solution) is an anti-parasitic eye drop used to treat a condition of the eye called Demodex blepharitis. Demodex blepharitis is a common inflammatory eye condition. It is caused by a type of parasite called Demodex mites, which are parasites that are commonly found on human skin. Demodex blepharitis may involve various parts of the eye, including the skin, eyelashes, eyelash follicles, and sebaceous glands. Symptoms may include red and crusty eyes, as well as the presence of eyelash collarettes, a type of dandruff that includes cells, mite eggs, and mite waste. Demodex blepharitis can only be diagnosed and treated by an eye doctor. Xdemvy was approved by the U.S. Food and Drug Administration (FDA) in July of 2023.

Xdemvy is not expected to be covered by commercial insurance plans until 2024 and Medicare Part D in 2025. When Xdemvy is covered by insurance, Xdemvy costs will vary by plan. Xdemvy is a new brand-name drug. There is no generic version of Xdemvy currently available.
Once patent forms for a new brand-name drug are filled out by the manufacturer, it can take up to about 20 years until generic versions may enter the market.
However, because there are several years of clinical trials, by the time the brand-name drug is approved by the FDA, there are generally about seven to 12 years left on a patent until a generic may become available.
Xdemvy is an eye drop that contains the active ingredient lotilaner. It is classified as an ectoparasiticide, or anti-parasitic, eye medication. More specifically, it is known as a gamma-aminobutyric acid (GABA)-gated chloride channel inhibitor.
Xdemvy treats an inflammatory eye condition called Demodex blepharitis. This condition is caused by mites that live on the human skin and can affect various parts of the eyes, including the skin, eyelashes, eyelash follicles, and sebaceous glands.
Common side effects (occurring in about 10% of people who use Xdemvy) include eye burning and irritation.
Less common side effects (occurring in about 2% of people who use Xdemvy) may include chalazion/hordeolum (styes) and punctate keratitis (corneal inflammation and damage).
Serious side effects are not expected. However, if you experience symptoms of an allergic reaction, such as hives, difficulty breathing, or swelling of the face, lips, tongue, or throat, get emergency medical help right away.

Xdemvy is available as an ophthalmic solution that contains lotilaner 0.25%.
Xdemvy is only approved for use in adults. The usual dosage is one drop instilled in each eye twice a day (about 12 hours apart) for six weeks.
Xdemvy contains a preservative called potassium sorbate, which may discolor soft contact lenses. Remove contact lenses before using Xdemvy. You can put them back in at least 15 minutes after instilling the eye drops.
While using Xdemvy, it is important to avoid contamination. Therefore, do not allow the tip of the bottle to touch the eye, anywhere around the eye, your fingers, or any other surfaces. Contaminated solution can cause serious eye damage, which may lead to vision loss.
If you use more than one topical eye medication, wait at least 5 minutes between using each medication.
While using Xdemvy, contact your doctor right away if you:
Develop any other eye conditions, such as infection
Require eye surgery
Develop any eye reactions like pink eye or eyelid reactions
This medication is not approved for use in children or adolescents under 18 years old. If you are pregnant or breastfeeding, consult your healthcare professional for medical advice.
Xdemvy should be stored at room temperature, away from direct light, heat, and moisture. Keep out of reach and out of sight of children. It can be used until the expiration date on the bottle.
