Hydrochlorothiazide is an FDA-approved generic prescription medication used to treat high blood pressure. It may also be used to treat swelling due to certain medical conditions or medication. Hydrochlorothiazide is a member of a class of drugs known as diuretics or water pills. It works by causing your body to make more urine. This helps in getting rid of extra salt and water from your body.

Hydrochlorothiazide comes in the form of an oral tablet or capsule and is usually taken once daily in the morning with or without food. Taking this medication before bedtime may cause you to wake up during the night to urinate, so it is usually recommended that you take this medication in the morning. Your doctor can advise you on the best time to take it and will prescribe an appropriate dose for you.

This medication should be used as prescribed in order to maximize its effectiveness. Continue using this medication even if you feel well. Most patients with high blood pressure usually do not feel sick. Do not take this medication in doses that are higher or lower than what was prescribed by your doctor.

What are the side effects of Hydrochlorothiazide?

Hydrochlorothiazide may lead to side effects such as an upset stomach, headaches, weakness, or dizziness. These effects may occur when you first start using this medication but usually go away as your body becomes accustomed to treatment. Talk to your doctor or pharmacist if you experience any of these effects and they persist or worsen.

Your doctor may also give you dietary instructions such as eating a low-salt diet, and/or taking potassium supplements, or increasing your intake of potassium-rich foods like bananas and orange juice. Follow these instructions while taking Hydrochlorothiazide.

Hydrochlorothiazide may cause you to lose too much body water which may lead to dehydration. Seek immediate medical attention if you experience symptoms of dehydration including extreme thirst, very dry mouth, muscle cramps, weakness, confusion, or decreased urination. 

Severe allergic reactions are rare but go to the emergency room for medical help if you have symptoms of an allergic reaction. These may include hives, difficulty breathing, or swelling around the face, lips, tongue, or throat. 

A rare but life-threatening skin reaction may occur, which is a medical emergency. These symptoms may include fever, burning eyes, sore throat, skin pain, red or purple rash, or blistering/peeling skin. If these symptoms occur, go to the emergency room. 

While taking Hydrochlorothiazide, call your physician right away if you have any of the following symptoms:

  • Lightheadedness

  • Eye pain or vision problems

  • Yellowing of the skin or whites of the eyes

  • Pale skin, easy bruising or bleeding

  • Shortness of breath, wheezing, mucusy cough, chest pain

  • Signs of an electrolyte imbalance such as increased thirst, increased urination, confusion, vomiting, constipation, muscle or bone pain, leg cramps, low energy, tingling, irregular heartbeat

Hydrochlorothiazide can cause sun sensitivity and an increased risk of certain types of skin cancer. Avoid tanning beds and sunlight while taking Hydrochlorothiazide. When you are outside, wear protective clothing and apply sunscreen SPF 30 or higher. Ask your healthcare provider about regular skin cancer screenings.

Who should not take hydrochlorothiazide?

Hydrochlorothiazide is not safe for everyone. Individuals who are allergic to Hydrochlorothiazide or any drug that contains Hydrochlorothiazide should not take this medication. Also, people who are allergic to sulfa drugs should not take Hydrochlorothiazide. Others who should not take Hydrochlorothiazide include:

  • Breastfeeding women (should not take more than 50 mg per day)

  • People with severe kidney problems or who have difficulty producing urine 

Hydrochlorothiazide should also be used cautiously in certain individuals, including:

  • Older adults

  • White patients (due to the risk of skin cancer)

  • People with liver or kidney problems

  • People who are volume depleted or who have electrolyte abnormalities

  • People with arrhythmia (irregular heartbeat)

  • People with diabetes

  • People with a seizure disorder

  • People with high blood pressure during pregnancy

  • People with lupus

  • People with a history of gout

  • People with a history of pancreas problems
