Gastroesophageal reflux disease (GERD) is caused when a muscle, the lower esophageal sphincter, relaxes and allows stomach contents, including acid, to flow up into the esophagus. The main symptoms of GERD are heartburn and chest pain.
Diabetic gastroparesis is a condition found in people who have had diabetes for a long time. The muscle that keeps food in the stomach from passing into the intestines, the pyloric sphincter, does not relax. Food backs up in the stomach. The main symptoms are nausea and vomiting.
The active ingredient in Reglan, metoclopramide, blocks neurotransmitters—chemicals that send signals between nerves—in the brain's “nausea center,” and relieves nausea and vomiting.
Metoclopramide and also increases a neurotransmitter that is vital to muscle contractions, so it produces better muscle tone in the lower esophageal sphincter, preventing food from backing up into the esophagus in patients with GERD as well as contributing to the relief of nausea and vomiting in patients with gastroparesis.
Reglan is for short-term use of up to 12 weeks. Do not take a higher dose of Reglan than what your doctor prescribed. Do not take Reglan for more than 12 weeks. Taking higher doses of Reglan or taking it for a long time increases the risk of a movement disorder that may not be reversible. People with diabetes and older adults, especially women, are at higher risk.
Reglan is not right for everyone. If you have or have had stomach or intestinal problems, a seizure disorder, depression, or an adrenal gland tumor, you should not take Reglan. If you have ever had muscle movement problems after taking Reglan or similar medications, or if you have ever had tardive dyskinesia (a movement disorder), you should not take Reglan.
Before taking Reglan, talk to your doctor about all of your medical conditions. Tell your doctor about all of the medicines you take, including prescription drugs, OTC medicines, and vitamins or supplements.
The most common side effects of Reglan include restlessness, drowsiness, tiredness, and lack of energy.
Serious side effects include a severe nervous system reaction resulting in stiff muscles and fast or uneven heartbeat.
Another serious side effect that Reglan can cause is called tardive dyskinesia, which is a sometimes irreversible condition that results in involuntary movements of the face and the rest of the body. Immediately discontinue Reglan and contact your doctor if you notice tremors, shaking, uncontrolled movements in your face (uncontrolled chewing, blinking, or eye movement), or any other uncontrolled movements in your lips, tongue, face, eyes, arms, or legs. Mental and mood changes may occur and include anxiety, agitation, and depression. People with depression should not take Reglan. Reglan may increase the risk of suicidal thoughts and behavior.
Allergic reactions are rare, but you should seek emergency medical help if you notice a rash, swelling, or start to have trouble breathing.
