Otrexup is a prescription injection prescribed to patients with rheumatoid arthritis, polyarticular (multiple joints) juvenile rheumatoid arthritis, or severe, debilitating psoriasis. In all three cases, Otrexup is only prescribed when other, first-line treatments have not succeeded.
All three are conditions in which the immune system misfires and targets healthy tissues as if they were infections. In psoriasis, the immune system targets skin tissues, and in rheumatoid arthritis, the immune system goes after cartilage tissues in joints. During active periods of psoriasis or rheumatism, the immune system's white blood cells divide, producing more white blood cells, congregate around the targeted tissues, and release chemicals that cause swelling and damage to the targeted tissues.
Otrexup is administered once per week as a subcutaneous injection, that is, it is injected beneath the skin of the abdomen or thigh. It is very important to only use the injection once per week. The dose will depend on the underlying condition.
Before taking Otrexup, tell your provider about all of the medical conditions you have, and all of the medications you take, including Rx and OTC drugs, vitamins, and supplements. While taking Otrexup, avoid alcohol, because it can increase the risk of liver problems. Do not drive until you know how this medication affects you. Avoid exposure to sunlight. You should not receive a live vaccine while taking Otrexup. Ask your healthcare professional before getting a vaccine.
Keep Otrexup out of the reach and out of sight of children and pets.
Severe side effects of Otrexup depend on the dose and length of treatment, but they include severe bone marrow suppression (putting the patient at risk of serious or fatal infections), liver damage, anemia, lung disease (which may not be reversible), lymphomas (cancer of the white blood cells), kidney problems, and gastrointestinal bleeding.
Otrexup is toxic to and can kill an unborn baby. Talk to your doctor about birth control to prevent pregnancy while taking Otrexup. Do not breastfeed while taking Otrexup.
Otrexup is not used to treat cancer, but if a patient taking Otrexup has a rapidly growing tumor, Otrexup can cause tumor lysis syndrome, a health-threatening imbalance in the body's electrolytes due to the death of cancer cells.
Common side effects of Otrexup include, but are not limited to, drowsiness, dizziness, nausea, vomiting, diarrhea, upset stomach, cold sores in the mouth, fever, chills, fatigue, and “feeling bad” (malaise).
Otrexup also reduces fertility: it lowers sperm counts in men and disrupts the menstrual cycle in women. It is not known if the infertility is reversible.
Otrexup also suppresses the body's immune system, so patients are more susceptible to infections. Consult your doctor immediately if you have an infection while being treated with Otrexup.
Allergic reactions are rare, but they potentially can be severe or life-threatening. These include Stevens-Johnson syndrome and toxic epidermal necrolysis, which are serious and potentially fatal skin reactions. Watch for any signs of an allergic reaction—fever, sore throat, flu symptoms, rash, or blisters—and immediately get emergency medical attention if you suspect you're having an allergic reaction.
