What is Disulfiram?

Disulfiram is a generic drug prescribed to people with chronic alcoholism to act as a chemical deterrent. Disulfiram is harmless when taken alone, but causes acute and painful side effects only a few minutes after someone taking Disulfiram drinks even a small amount of alcohol: pounding headache, trouble breathing, fast heartbeat, low blood pressure, nausea, and severe vomiting. A severe Disulfiram-alcohol reaction can be fatal.

Disulfiram works by interfering with the body's ability to completely metabolize alcohol, leaving instead a fairly toxic chemical, acetaldehyde, to build up in the body. The effects kick in only a few minutes after the patient drinks alcohol and typically last for several hours.

Disulfiram is often started in a hospital or rehabilitation facility. Disulfiram should never be taken when the patient is drinking alcohol. The treatment should be started at least 12 hours after the last alcoholic beverage. The patient must be fully aware of the Disulfiram-alcohol reaction, and understand that drinking alcohol can cause severe reactions. The family and caregivers should also be educated. Alcohol or products containing alcohol cannot be ingested, this includes hidden sources of alcohol such as mouthwash, cough syrups, foods with alcohol (such as sauces), and even topical preparations like back rubs or shaving products. Disulfiram should not be taken with metronidazole. Disulfiram has other drug interactions too, so review all medications (including Rx, OTC, vitamins, and supplements) with your doctor before starting this drug. Disulfiram is active in the body for several weeks, so patients will not be able to drink alcohol for at least two weeks after discontinuing Disulfiram. While taking Disulfiram, the patient must carry a medical card that states they are taking Disulfiram. This lets emergency responders know that if the patient is disoriented or unconscious, they may be having a Disulfiram reaction. The card also tells responders not to administer alcohol or any preparations that contain alcohol, and lists supportive measures that can be used, such as oxygen inhalation. 

What are the side effects of Disulfiram?

The side effects of Disulfiram are minimal when taken alone and rarely experienced: drowsiness, skin rash, pimples, and a bad taste in the mouth.

When Disulfiram is taken with alcohol, side effects are unpleasant and potentially life-threatening: nausea, severe vomiting, neck pain, throbbing headache, fast heartbeat, fluttering heartbeat, sweating, thirst, shortness of breath, confusion, blurred vision, and chest pain. The Disulfiram-alcohol reaction can cause cardiovascular effects such as severe chest pain, low blood pressure, and heart failure or a heart attack. The Disulfiram-alcohol reaction also slows and may stop breathing or cause death.
